ارائه و تبیین مدل ترک خدمت کادردرمان در زمان اپیدمی کووید19 (مورد مطالعه: بیمارستان های دولتی شهر تهران)

چکیده

هدف: هدف اصلی این مقاله، ارائه و تبیین مدل ترک خدمت کادر درمان در زمان همه‌گیری کووید 19 در بیمارستان‌­های دولتی شهر تهران می‌باشد.
طراحی/روش‌شناسی/رویکرد: پژوهش حاضر از جمله تحقیقات ترکیبی می­‌باشد که بخش کیفی آن با استفاده از روش تحلیل مضمون و متعاقب آن بخش کمی بر اساس الگوی شناسایی شده در بخش کیفی انجام شد. در بخش کیفی از مبانی نظری موضوع و مصاحبه‌­های نیمه ساختارمند استفاده شد. مصاحبه‌­ها به صورت هدفمند و با روش گلوله برفی با خبرگان نظیر مدیران منابع انسانی و پزشکان متخصص و سرپرستاران با تجربه صورت گرفت و در بخش کمی پرسشنامه‌­ای بر اساس الگوی استخراج شده طراحی و با میزان پایایی مطلوب (ضریب آلفای کرونباخ بالاتر از 0.7) توسط اعضای کادر درمان بیمارستان­های دولتی شهر تهران تکمیل شد. تعداد افراد جامعه 14000 نفر برآورد گردید که تعداد نمونه با توجه به تعداد متغیرهای مکنون (9 عدد) و مضمون فرعی (26 سوال) در سطح خطای 0.05درصد 1960 نفر تعیین و روش نمونه‌­گیری به صورت خوشه­‌ای اجرا شد.
یافته‌­های پژوهش: در نتیجه تحلیل داده‌­ها در بخش کیفی 50 کد گزینشی، 26 مضمون فرعی و 9 مضمون اصلی شناسایی شد. در بخش کمی مدل مورد نظر دارای برازش مناسب بوده و تمامی فرضیه­ها مورد تایید قرار گرفت.
محدودیت­ها و پیامدها: بخش کیفی مبتنی بر پارادایم تفسیری است و در این بخش ذهنیت پژوهشگران و پیش فرض‌های آنان در استخراج مفاهیم تأثیرگذار بوده است. در بخش کمی نیز تعداد بالای اعضای مورد پژوهش و ماهیت مقطعی داده‌­ها جمع‌آوری اطلاعات را بسیار دشوار کرده است.
پیامدهای عملی: ترک خدمت کادر درمان در زمان اپیدمی کووید 19 از معضلات مراکز درمانی کشور می­‌باشد که منجر به نارضایتی بسیاری از بیماران می­‌گردد، از همین رو شناخت عوامل ترک خدمت کادر درمان کمک شایانی به افزایش کیفیت درمان از سوی مراکز درمانی می­‌نماید.
ابتکار یا ارزش مقاله: این پژوهش با ارائه و تبیین مدل ترک خدمت کادر درمان در زمان اپیدمی کووید 19، در بیمارستان­های دولتی شهر تهران به شناسایی عوامل جدیدی در ترک خدمت کادر درمان کمک کرده است.

چکیده [English]

Purpose: The main purpose of this article is to identify the components of dismissal of medical staff in the event of an epidemic of viral diseases (Covid 19) and its fitness and explanation in the community of medical staff of public hospitals in Tehran.
Design/Methodology/Approach: The present study is one of the combined researches, the qualitative part of which was done using the content analysis method and then the quantitative part based on the pattern identified in the qualitative part. In the qualitative section, the theoretical foundations of the subject and semi-structured interviews were used. The interviews were conducted purposefully by snowball method with experts such as Miran HR and improving the quality of hospitals and experienced doctors and nurses. Completed, the number of people in the community is estimated at 14,000. The number of samples was determined according to the number of latent variables, which were 9 and the number of questions of 26 sub-themes at an error level of 0.05% of 1960, and the sampling method was cluster sampling.
Research Findings: As a result of data analysis, 50 selection codes, 26 sub-themes and 9 main themes were identified, which in a quantitative way was confirmed by the method of structural equation validity and accuracy of the obtained model.
Limitations & Consequences: The qualitative part is based on the interpretive paradigm and in this part, the mentality of researchers and their assumptions was effective in extracting principles. In a small part, the large number of members under study and the cross-sectional nature of the data make it very difficult to collect information.
Practical consequences: Medical staff leaving the service during the covid-19 epidemic is one of the problems of the country's medical centers, which leads to the dissatisfaction of many patients, therefore, knowing the factors of medical staff leaving the service helps to increase the quality of treatment from the medical centers.
The innovation or value of the article: This research has helped to identify new factors in the medical staff leaving service in public hospitals of Tehran by presenting and explaining the model of leaving the service of the medical staff during the covid-19 epidemic.
